Team Participation Acknowledgment & Media Authorization

Participation and liability. I understand that lacrosse and related league activities involve inherent risks of injury, loss, or damage. I voluntarily accept those risks for myself and the registered team and, to the fullest extent permitted by law, release and hold harmless The Sixes League, its owners, operators, facilities, staff, coaches, volunteers, and partners from claims arising from participation, except for liability that cannot legally be waived.

Photo and video authorization. I authorize The Sixes League to photograph or record the registered players and to use their images, likenesses, and video on league websites, social media, and promotional materials without compensation.

I certify that I am authorized to register this team and have obtained permission from each player’s parent or legal guardian to accept this acknowledgment and media authorization on their behalf.

Registration fees and refunds. All registration fees are final and non-refundable, except where required by law.

A submission notice will be sent to thesixesleague@gmail.com, and the roster will be archived in our Team Registrations Drive folder. You’ll receive a confirmation email, and the next screen includes the secure Boys and Girls QuickBooks payment links.
